The PTFA will be producing a ‘Leavers T-shirt’ for all Year 6 children, which will feature the children’s names and will be given as a gift to each child at the end of the Summer Term. To comply with GDPR guidelines, you will need to give the PTFA permission, via this form, to print your child’s name. If you do not return the form or do not give your consent, your child’s name will not be printed.  

When completing this form please ensure your child's name is spelt correctly and without errors, as this will be the name used for the T-shirt, should you provide consent. Your child will still receive a t-shirt if you do not give consent, though without their name.  This form needs to be completed by 10th May in order for your child to appear on the t-shirt.
